Allergic Sinusitis

A 52-year-old woman was diagnosed with chronic sinusitis

She had history of atopy and asthma required corticosteroids

She had eosinophilia and increased serum IgE

She underwent bilateral endoscopic maxillotomy & ethmoidectomy

Nasal tissue was sent to microbiology for C&S

Initial Gram stain results were reported as yeast from microbiology

Further testing of yeast looking cells revealed consistency with mould

Culture grew fungus identified as Trichoderma species

Speciated as Trichoderma longibracheatum by UAMC & Herbarium
