Phaeohyphomycosis
• A leg mass specimen
was received from a middle age male patient
• Specimen was requested
for C&S only
• Gram stain results
were reported as pus cells seen, no bacteria seen
• However pale staining
filaments were observed, indicating dark fungus
• Specimen was retrieved
and processed for fungal culture
• One week later,
the culture grew a dark mould, yeasty in consistency
• LPAB from the slide
culture identified as Exophiala species
• Exophiala jeanselmei
was confirmed by the reference laboratory
• The specimen was
requested for C&S only, the fungus could have been missed
if not picked up by Gram stained smear reader
