Phaeohyphomycosis

A leg mass specimen was received from a middle age male patient

Specimen was requested for C&S only

Gram stain results were reported as pus cells seen, no bacteria seen

However pale staining filaments were observed, indicating dark fungus

Specimen was retrieved and processed for fungal culture

One week later, the culture grew a dark mould, yeasty in consistency

LPAB from the slide culture identified as Exophiala species

Exophiala jeanselmei was confirmed by the reference laboratory

The specimen was requested for C&S only, the fungus could have been missed if not picked up by Gram stained smear reader
