Culture positive for Partial Acid-Fast by Modified Kinyoun (MK)

• 51 year old male came to the ER with swelling knee

• Initial diagnosis arthritis, treated with prednisone

• Patient had stroke, went off balance and fell on knee

• A knee aspirate was sent to microbiology for C&S

• Small, pale, whitish & gritty colonies grew after 3 days

• MK stain was positive & Nocardia results were reported

• Further testing identified the organism as fungus

• Gram smear was reviewed
