lung transplant patient came for follow up
• A BAL specimen
was submitted to the microbiology lab
• Gram stain results
were reported as no bacteria seen
• Fungal smear showed
round cells, large to medium sized, single and non-budding
• Structures resembled
immature spherules
• Original Gram smear
was reviewed that showed large, thick-walled budding yeast with
broad base
• Subsequent fungal
smears, GMS & PAS confirmed the structures as Blastomyces
dermatitidis
