Post Lung Transplant Patient

A female, post lung transplant patient came for follow up

A BAL specimen was submitted to the microbiology lab

Gram stain results were reported as no bacteria seen

Fungal smear showed round cells, large to medium sized, single and non-budding

Structures resembled immature spherules

Original Gram smear was reviewed that showed large, thick-walled budding yeast with broad base

Subsequent fungal smears, GMS & PAS confirmed the structures as Blastomyces dermatitidis
