ICU Influenza Surveillance
Proposal – October 2006

» Objectives:

    1. to describe the incidence, clinical features and outcomes of laboratory-confirmed influenza illness requiring ICU admission.

    • At present we do not know the number of patients that are admitted to ICU due to influenza and its complications, or what the clinical features of their disease are
    2. to assess the effect of treatment with oseltamivir (tamiflu) on length of stay, length of ICU stay and death in cases of laboratory confirmed influenza requiring ICU admission.

» How the surveillance works:

    Diagnostic tests for influenza (rapid antigen, PCR and culture) from nasopharyngeal swabs (NP) will be performed on patients admitted to the ICU with the following eligible diagnosis:

    • acute cardio-respiratory illness (including but not limited to pneumonia)
    • infection/sepsis of unknown origin


» Benefits for ICUs participating in the study:

    The results of this investigation may help determine the actual number of hospitalized influenza cases and determine if routine influenza testing is beneficial. Participating ICUs will have rapid antigen and PCR testing available for diagnosing influenza in the 2006/7 season; if this is of benefit to patients, then patients in participating ICUs may benefit.

 

» Added work for ICUs participating in the study:

    Staff in participating ICUs will be asked to :

    • Assess patients to see if they meet the eligibility criteria
    • Perform an NP swab on patients who meet these criteria
    • Send NP swab to lab

» Confidentiality:

  • Results will be treated like all other lab results for individual patients.
  • All test results will be grouped for analysis
  • Each participating hospital will receive their grouped data
  • Data identifying individual hospitals or patients will not be released
  • This study has been approved by the Ethics Review Boards of all participating hospitals
